Interactions

Ask your doctor for more details. Without your doctor's permission, you should never start, stop, or modify the dosage of any medications. Prednisone shouldn't be used to avoid rashes because, for the first six weeks of nevirapine therapy, it may actually make them more likely. Drug interactions may change how your medications work or increase your risk for serious side effects. Rifamycins (like rifampin) are just a few examples. Antiarrhythmics (like amiodarone), asunaprevir, cobicistat, elvitegravir, some medications used to treat seizures (like clonazepam), azole antifungals (like itraconazole, ketoconazole), macrolide antibiotics (like clarithromycin), and methadone are a few examples of affected medications. John's wort, among others. Keep a list of everything you use, including herbal products, prescription and over-the-counter medications, and give it to your doctor and pharmacist. Nevirapine can expedite the removal of many other medications from your body, which may impact how well they function. The removal of nevirapine from your body can be impacted by other medications, which could change how nevirapine functions. Some other drugs to treat HIV infection (including protease inhibitors such as atazanavir, ritonavir) may also interact with nevirapine. Some products that may interact with this drug include: orlistat, rifabutin, warfarin. Use an effective barrier method (such as latex or polyurethane condoms or dental dams) during sexual activity to reduce the risk of HIV transmission to others, and discuss additional or alternative reliable methods of birth control with your doctor. If you notice any new spotting or breakthrough bleeding, let your doctor know as these could indicate that your hormonal birth control isn't working as intended. Pregnancy could result from this. Not every possible drug interaction is covered in this document. The effectiveness of hormonal birth control methods like pills, patches, or rings may be reduced by this medication. To lower the chance of side effects, your doctor will modify your medications and oversee your therapy.

HDL-cholesterol increased by 43% in patients on nevirapine and 34% on efavirenz (p = 0.036). Patients starting with low or normal HDL cholesterol at baseline had increases, whereas those with high HDL cholesterol had slight decreases. However the increase in HDL and total cholesterol was greater (p < 0.001) than in LDL-cholesterol (p = 0.018). There are two main types of cholesterol: low-density lipoprotein (LDL) or ‘bad’ cholesterol (which may put people at risk for heart disease and other serious conditions), and high-density lipoprotein (HDL) or ‘good’ cholesterol (which helps get rid of LDL). Regional differences in HDL changes were also evident, say the investigators, although data are not reported.

After adjusting for blood lipid levels, these increased risks were slightly reduced. At last year’s CROI, they reported the controversial finding that recent use (within six months) of the NRTI abacavir (Ziagen, also in the Kivexa and Trizivir coformulations) appeared to increase the risk of myocardial infarction (MI), or heart attack, by 90%, whilst ddI (didanosine, Videx) increased this risk by 49%. The investigators found no association between heart attack risk and recent or cumulative exposure to either of the studied NNRTIs, two of the PIs (nelfinavir and saquinavir), or most of the N(t)RTIs. However, they did see a significant association between higher MI risk and cumulative - but not recent - exposure to indinavir and lopinavir/ritonavir.
