Dosage

Over the course of three days, a total dose of 25 mg of base/kg is administered as follows: 10 mg base/kg as the first dose, followed by 5 mg base/kg six hours later, 5 mg base/kg 18 hours later, and 5 mg base/kg 24 hours later. Pay close attention to the instructions on your prescription label. It is preferable to administer medications to children (6 years of age and older) based on their body weight. Lupus Erythematosus The recommended initial dosage of hydroxychloroquine (Plaquenil) for the treatment of lupus erythematosus is 400 mg once or twice daily. Adults should take 400 mg of hydroxychloroquine (Plaquenil) on the exact same day each week to suppress malaria. Two weeks prior to exposure, suppressive therapy should start. Take this medication exactly as prescribed by your doctor. The dosage for adults may also be determined by body weight. The dosage your doctor prescribes for you may depend on the following factors: the condition being treated; other medical conditions you have; other medications you are taking; how you respond to this medication; your weight; your age; your kidney function; and your liver function. Rheumatoid Arthritis The initial dosage of hydroxychloroquine (Plaquenil) is recommended for the treatment of rheumatoid arthritis and ranges from 400 to 600 mg per day. For the treatment of lupus erythematosus, hydroxychloroquine (Plaquenil) maintenance doses of 200 to 400 mg per day are advised. The recommended maintenance dose range of hydroxychloroquine (Plaquenil) for the treatment of rheumatoid arthritis is 200 to 400 mg daily. For children aged 6 and older, the weekly suppressive dose of hydroxychloroquine (Plaquenil) is 5 mg base/kg. For eight weeks after leaving the endemic area, the suppressive therapy should be continued. Regardless of body weight, this dose should not go over the recommended adult dosage. On each of the following two days, 400 mg are then taken. Adults who have malaria are advised to take 800 mg of hydroxychloroquine (Plaquenil), followed by 400 mg in six to eight hours. Give an initial loading dose of 800 mg to adults when it is not given prior to exposure, or 10 mg base/kg to children when given in two divided doses spaced six hours apart.

Missed dose

If you miss a dose, call your doctor for instructions.

Overdose

Call your doctor or the local poison control center immediately if you take too much Plaquenil, or go to the hospital for immediate emergency care.

Side effects of antimalarials Most people don’t notice any side effects, but antimalarials sometimes cause stomach pain and digestive problems, like nausea or diarrhea. It usually takes 1 to 3 months to start working. Doctors use antimalarials to treat malaria, but these medicines can also treat lupus by: Reducing pain and inflammation Preventing lupus flares and helping with lupus skin problems Lowering the dose (amount you need to take) of your other lupus medicines Most people with lupus take hydroxychloroquine throughout their lives. Hydroxychloroquine may also help prevent blood clots and organ damage from lupus. Types of antimalarials Hydroxychloroquine (Plaquenil) is the most common antimalarial for lupus.

It’s been used since the 1940s to prevent and treat malaria, and to treat rheumatoid arthritis and lupus. It’s sold in generic form and under the brand name Plaquenil in the United States. Doctors also can prescribe it off label for other purposes, as many are doing now for COVID-19. Some politicians and doctors are sparring over whether to use Hydroxychloroquine against the new coronavirus, with many scientists saying the evidence is too thin to recommend it now.

My husband and I spent an afternoon moving him into my office. I texted my brother and sister, explaining what I just witnessed. For a moment, I caught a glimpse of our new normal and had a sense it might be okay.
