Fatal hepatotoxicity and pancreatitis have been noted in patients undergoing therapy with lopinavir and patients with an increased baseline risk of these events should be monitored closely throughout therapy. Given its moderate duration of action, once or twice daily dosing is required. An enzyme essential to the HIV viral lifecycle is inhibited by lopinavir. When giving lopinavir to patients taking other medications, use caution as pharmacodynamic and pharmacokinetic interactions are frequently observed. Lopinavir, like other protease inhibitors, has a tendency to participate in drug interactions.

Observe St. Co-administering St. John's wort to induce CYP3A John's Wort may result in reduced drug plasma concentrations and therapeutic failure. St. John's Wort take while eating. While the tablet formulation can be taken either way, the oral solution formulation must be taken with food.

Study participants were predominantly male (79%). There are two main types of cholesterol: low-density lipoprotein (LDL) or ‘bad’ cholesterol (which may put people at risk for heart disease and other serious conditions), and high-density lipoprotein (HDL) or ‘good’ cholesterol (which helps get rid of LDL). TITAN compared twice daily darunavir/ritonavir (600/100mg) with twice daily lopinavir/ritonavir capsules (the old formulation) (400/100mg), plus a background regimen selected by resistance testing. Around 30% had no previous experience of protease inhibitor treatment. This trend suggests that darunavir is intrinsically more potent, due to its greater barrrier to developing resistance mutations.
