Contraindications
Treatment should be discontinued if azotemia worsens. During treatment with aminoglycosides, renal function monitoring is crucial. Patients should drink plenty of water to prevent chemical irritation of the renal tubules because amikacin is found in high concentrations in the renal excretory system.
It causes ototoxicity, which is why it is no longer used in humans. It is a semisynthetic aminoglycoside antibiotic used in the treatment of tuberculosis. This leads to the synthesis of defective, nonfunctional proteins, which results in the bacterial cell's death. It acts by irreversibly binding the S12 protein in the bacterial 30S ribosomal subunit, after being actively transported across the cell membrane, which interferes with the initiation complex between the mRNA and the bacterial ribosome.

Concomitant therapy with other anti-infective agents (e.g., b-lactam antibiotics) was administered in most studies of once-daily aminoglycoside therapy confounding accurate determination of the efficacy of the aminoglycoside alone as once-daily therapy. Oral administration of kanamycin or neomycin is contraindicated in patients with intestinal obstruction. Some strains of enteropathogenic E. coli are resistant to neomycin; therefore, prior to initiation of neomycin therapy appropriate specimens should be collected for culture and susceptibility tests. Tobramycin solution for oral inhalation is administered via a nebulizer; tobramycin and gentamicin also have been administered by oral inhalation as aerosols prepared extemporaneously from parenteral preparations of the drugs.
The cimetidine injectable product is compatible with the commonly used IV infusions solutions, including amino acid (TPN) solutions, but should be used within 48 hours of dilution.
