Description

An antibiotic known as an aminoglycoside, amikacin sulfate is semi-synthetic. Amikacin is active in vitro against pseudomonas species, Escherichia coli, Proteus species, Providencia species, Klebsiella-Enterobacter species, Acinetobacter species, and Citrobacter freundii. In vitro, amikacin sulfate is effective against methicillin-resistant strains of Staphylococcus as well as species that do not produce penicillinase. The intramuscular route is preferred for the majority of infections, but the intravenous route may be used in patients for whom the intramuscular injection route is impractical or for whom the infection is life-threatening. Amikacin may be administered intraperitoneally in a concentration of 0.25% after waking up from anesthesia. Many of the above-mentioned organisms' strains are susceptible to Amikacin even when they are resistant to other aminoglycosides like Gentamicin, TobrAmykin, and KanAmykin.

Dosage

A maximum total adult dose of 15g should not be exceeded. Adults and children: 15 mg/kg/day in two equally-divided doses (equivalent to 500 mg bid in adults). Elderly: Amikacin is eliminated through the kidneys. Impaired renal function: To prevent drug accumulation in patients with impaired renal function, the daily dose should be lowered and/or the time between doses should be lengthened. Life-threatening infections and/or infections brought on by Pseudomonas: The adult dose may be increased to 500 mg every eight hours, but it should not exceed 1.5g/day or be given for more than 10 days. Neonatals and premature infants should receive a loading dose of 10 mg/kg, followed by two doses of 15 mg/kg/day, given equally spaced apart. When possible, renal function should be evaluated, and dosage should be adjusted as per the information under impaired renal function. Urinary tract infections (other than pseudomonal infections): 7.5 mg/kg/day in two equally divided doses (equivalent to 250 mg b.i.d. in adults). Use of the 100 mg is recommended for children for the accurate measurement of the appropriate dose.

Especially tell your doctor if you take: acyclovir (Zovirax, Sitavig) amphotericin (Abelcet, Ambisome, Amphotec) bacitracin capreomycin (Capastat) certain cephalosporin antibiotics such as cefazolin (Ancef, Kefzol), cefixime (Suprax), or cephalexin (Keflex) cisplatin (Platinol, Platinol-AQ) colistin (Coly-Mycin S) cyclosporine (Gengraf, Neoral, Restasis, Sandimmune) diuretics ('water pills') such as bumetanide, ethacrynic acid (Edecrin), furosemide (Lasix), or torsemide (Demadex) other aminoglycoside antibiotics such as gentamicin, kanamycin, neomycin (Neo-Fradin), paromomycin, streptomycin, or tobramycin polymyxin B vancomycin other antibiotics such as amoxicillin (Amoxil, Moxatag, in Augmentin, in Prevpac), ampicillin, or penicillin dimenhydrinate (Dramamine) meclizine (Bonine) nonsteroidal anti-inflammatory drugs such as indomethacin (Indocin) This is not a complete list of amikacin drug interactions. Inform your physician of all the drugs you take, including vitamins, herbal supplements, prescription and non-prescription medications.

Contraindications

Treatment should be discontinued if azotemia worsens. During treatment with aminoglycosides, renal function monitoring is crucial. Concomitant therapy with other anti-infective agents (e.g., b-lactam antibiotics) was administered in most studies of once-daily aminoglycoside therapy confounding accurate determination of the efficacy of the aminoglycoside alone as once-daily therapy. Oral administration of kanamycin or neomycin is contraindicated in patients with intestinal obstruction. Some strains of enteropathogenic E. coli are resistant to neomycin; therefore, prior to initiation of neomycin therapy appropriate specimens should be collected for culture and susceptibility tests. Tobramycin solution for oral inhalation is administered via a nebulizer; tobramycin and gentamicin also have been administered by oral inhalation as aerosols prepared extemporaneously from parenteral preparations of the drugs.
