Contraindications
Call your doctor right away if you are experiencing chest pain. Chest pain may result from lowering the dosage of a beta blocker while taking nifedipine (see the "Drug Interactions" section for examples of beta blockers). Avoid abruptly ceasing the use of beta blockers if you have gastrointestinal (stomach and bowel) blockage. You could experience fainting or dizziness if your blood pressure is low, or hypotension. Inadequate fluid intake, excessive sweating, diarrhea, or vomiting can lead to an excessive fall in blood pressure, too. If you're feeling lightheaded or faint, lie down. Nifedipine has been associated with serious side effects, such as congestive heart failure (CHF). In very rare instances, small chunks of partially digested or undigested material can lodge in the GI tract and may need to be surgically removed to prevent gastrointestinal ulcers. Any of the following CHF symptoms should be reported to your doctor right away: sudden weight gain, worsening shortness of breath, increased swelling of the feet, legs, or abdomen, need for additional pillows or sleeping in a recliner, awakening from sleep to catch your breath, cough that won't go away, new or worsening irregularities in your heart rate, hypotension. You should not take nifedipine if you have any of the following conditions: an allergy to nifedipine or any of the medication's ingredients have a condition known as "essential hypertension," a type of high blood pressure. Tell your healthcare provider right away if you have some or all of the following symptoms of angina: a pressure, squeezing, burning, or tightness in the chest pain in the chest, breastbone area, arms, neck, shoulders, or back nausea fatigue shortness of breath light headedness weakness heart attack. If you experience any of the following heart attack symptoms, contact your doctor right away or dial 911: chest discomfort (pressure, squeezing, or pain); shortness of breath; discomfort in the upper body (arms, shoulder, neck, or back); nausea, vomiting; dizziness; lightheadedness; sweating; beta-blocker withdrawal. This kind of high blood pressure has an underlying cause that is unknown. Nifedipine's extended release form can result in stomach or small intestine ulcers, which are sores. Rare reports of this happening in people who had gastrointestinal structural abnormalities after taking nifedipine extended release have been made. Nifedipine use has the potential to increase the frequency or severity of heart attacks, especially in people who already have coronary artery disease, which is characterized by hardening and narrowing of the blood vessels supplying the heart. Nifedipine use is associated with an increased risk of or severity from heart attacks, particularly in people with coronary artery disease (a condition in which the blood vessels that supply the heart become hardened and narrowed). How much beta blocker you should take will be decided by your doctor.
Her blood pressure was kept under control throughout the pregnancy with labetalol (sold as Trandate, among other names) plus nifedipine (brand names include Adalat), which was added in the second trimester. Additionally, her blood cortisol levels failed to drop after an overnight dexamethasone suppression test (ODST), usually given to confirm the presence of hypercortisolism (excessive cortisol levels). One year after delivery, another pituitary adenoma was found on an MRI scan and she underwent a second transsphenoidal surgery. Labor was induced at 37 weeks of gestation due to cervical insufficiency — meaning the uterine cervix has opened (dilated) earlier than it is usual. The report, “Cushing Disease Treated Successfully with Metyrapone During Pregnancy,” was published in the journal AACE Clinical Case Reports.
Nifedipine belongs to a class of medications known as calcium channel blockers. Swallow the whole or split tablet without crushing or chewing.Your doctor may gradually increase your dose. Also, do not split the tablets unless they have a score line and your doctor or pharmacist tells you to do so.
After 48 weeks, people who were assigned to switch to the two-drug regimen had a similar rate of viral suppression to those who remained on the three-drug regimen. People with pre-existing liver problems, including hepatitis B or hepatitis C infection, had a greater risk of developing such a complication. Dosing for children is determined by the child’s weight; the minimum weight required for treatment with darunavir/ritonavir is 10kg. Neither lopinavir/ritonavir nor saquinavir is recommended for use with darunavir because of a significant decrease in darunavir serum concentration that occurs when these drugs are used together.
